Office Hours:
Monday - Thursday 7:30 am to 4:30 pm Friday
7:30 am to 3:30 pm The first appointment is
at 8:00 am and last appointment (Mon-Thurs) is seen
at 3:30 pm. The last appointment on Friday is seen
at 2:30 pm. A doctor is on call each evening
and every weekend, and holiday.

First Visit:
| Upon arriving for your first
visit in our office you will be required to
complete and sign multiple forms. To assist with
the process, we have posted copies of these
forms below, so that you can read and
print them in advance
before arriving at our offices. |

Payment Options:
- Payment in Full- 5% discount for cash/check, 3%
discount for credit card (Visa, MasterCard, Discover
or American Express)
- Split Payment - pay 1/2 at the time of service
and leave form of payment for the balance to be
posted/paid in 30 days.
- Care Credit - 6 months Same-as-Cash or $1500
or more qualifies for 12 months same as cash.

After Hours Emergency:
Our office phone is answered at
all times. Should an after hours endodontic
emergency arise that requires contacting our
office, please call - Cool Springs 615.371.9559
or Green Hills 615.383.4455. Follow the
instructions provided in the message. One of
our doctors will be contacted, and your call
will be returned as soon as possible. An
emergency is considered to be swelling, trauma,
or pain not controlled by medicines.
If
it is determined that an emergency office visit
will be required the doctor will contact
you to discuss the appointment time and offer
directions to our office.
An additional
fee will be added for after hours and weekend
emergency treatment.
After hours prescriptions
can only be issued to patients that we have
seen. It is unlawful for our office to
issue prescriptions (antibiotics, pain
medications, etc.) for patients that have not
been seen in our office. If you are a new
patient that we have not seen, please contact
your family dentist for
after hours or weekend prescriptions. If you
need a refill on any prescriptions, please call
during our regular office hours. |

Endodontic Associates is fully
committed to compliance with HIPPA guides in the
appropriate security of our patient records.
The misuse of Personal Health Information
(PHI) has been identified as a national problem
causing patients inconvenience, aggravation, and
money. We want you to know that all our
employees, managers and doctors continually
undergo training so that they may understand and
comply with government rules and regulations
regarding the Health Insurance Portability and
Accountability Act (HIPPA) with particular
emphasis on the PRIVACY RULE. We strive to
achieve the very highest standards of ethics and
integrity in performing services for our
patients.
It is our policy to properly
determine appropriate use of PHI in accordance
with governmental rules, laws, and regulations.
We want to ensure that our practice never
contributes in any way to the growing problem of
improper disclosure of PHI. As part of this
plan, we have implemented a Compliance Program
that we believe will help us prevent any
inappropriate use of PHI, therefore before we
release any medical or insurance records we
would request that you sign one of the
appropriate release forms.

We also know
that we not perfect! Because of this fact,
our policy is to listen to our employees and our
patients without any thought of penalization. If
they feel that an event in any way compromises
our policy of integrity, please let us know. More so, we
welcome your input regarding any service problem
so that we may remedy the situation promptly.
